Enforce certain default nodes in schema

I’d like to have a schema that enforces an H1 Title at the top of the page followed by whatever else the user wants. What is the best way to go about this? How can I disable block formatting commands from being available when that first block is selected?

This is what issue #220 tracks. It is my main focus right now, but interacts with a bunch of things in complex ways, so it’s prompted a major refactor which I’m currently doing in a separate branch. With luck, I’ll have a clearer roadmap in a few weeks.
